Preventive Botox®: Can Starting Early Help Reduce Wrinkles Long-Term?

Back in 2002, the FDA approved Botox® for cosmetic use, and the aesthetic world has been exploring the many fantastic anti-aging roles this cosmetic injectable can play ever since. 

Botox is widely recognized as one of the most effective ways to reduce the appearance of wrinkles around your forehead, brows, eyes and neck. This explains why nearly 10 million neurotoxin treatments were performed in the United States in 2024 alone.

But the uses of Botox are ever-expanding, and they now include a wrinkle-prevention role. To explain this, we turn to board-certified dermatologist Dr. Juan-Carlos Caballero and our team at Warrenton Dermatology & Skin Therapy Center, who have extensive experience with Botox.

Botox and dynamic wrinkles

To understand how Botox can be used as a preventive anti-aging treatment, we first need to understand how the cosmetic injectable works to smooth out wrinkles and fine lines.

Botox works best on what we call dynamic wrinkles, or wrinkles that come and go with your expressions (e.g., when you smile, frown, or squint). Dynamic wrinkles are formed by small muscles that contract and, when you’re done with the expression, your muscles relax and your skin smooths out again. 

At least this is what occurs before aging starts to take hold and the lines become slowly etched into your skin. With Botox, we inject a neurotoxin into these muscles that temporarily “quiets” them. As a result, the muscles don’t contract, and wrinkles don’t form.

Using Botox to prevent wrinkles

Given what we’ve just learned about how Botox works, it makes sense that we can use the cosmetic injectable proactively. After all, if you stop a dynamic wrinkle from forming over and over in the first place, you can prevent lines from becoming etched into your skin as you age.

To give you an idea about effective this can be, one study on using Botox as a preventive measure concluded that “earlier intervention may delay the onset of visible aging signs by modulating muscular activity [and] preserving skin elasticity.”

When is the right time to start Botox?

If you want to use Botox to manage aging in your face, deciding when to start is personal. We have clients who receive regular Botox injections in their twenties and thirties, which is a great time to proactively address aging. Even so, starting in your forties can also be effective.

A lot also depends upon your skin type and whether you inherited skin that stands up to time better, or skin that ages more prematurely. Your lifestyle matters, as well. If you spend a lot of time in the sun, photoaging can occur, which makes preventive Botox a great idea — the earlier, the better.
